Pan-fried liver smothered with caramelized onions and gravy is a nutrient-dense Southern family dinner rich in iron and protein. This classic, old-fashioned dish is bold in flavor and deeply nostalgic.
Slice the beef liver into ½-inch thick strips and soak them in whole milk for at least 30 minutes to reduce bitterness.
While the liver soaks, peel and slice the yellow onions thinly. Melt 2 tbsp unsalted butter in a large skillet over medium heat, then add the onions. Caramelize the onions slowly, stirring every few minutes until golden brown (about 15 minutes).
Remove the onions from the skillet and set aside. Pat the liver dry and dredge the slices in all-purpose flour seasoned with salt, black pepper, and garlic powder.
Add the remaining 2 tbsp unsalted butter to the skillet over medium-high heat. Place the floured liver slices in the hot skillet and sear for 2–3 minutes per side, until browned but still tender. Avoid overcooking to prevent toughness.
Pour in beef broth and return the onions to the skillet. Reduce heat to low and simmer for 5–7 minutes to allow flavors to meld and create a savory gravy. Serve hot, smothered with onions and buttery pan gravy.
