Add whole milk to a heavy-bottom saucepan. Bring to a simmer over medium heat, stirring occasionally to prevent scorching.
Reduce the heat to medium-low and continue simmering, stirring frequently, until the milk has reduced by about half and turned slightly creamy (about 35-40 minutes).
Stir in sweetened condensed milk, heavy cream, and sugar; cook gently for another 10 minutes on low heat, mixing thoroughly.
Add finely chopped pistachios and ground cardamom. Cook for 3-4 minutes more, then remove from heat. Stir in rose water.
Cool the mixture completely to room temperature, pour into kulfi molds or small cups, and freeze for at least 6 hours or until firm before serving.
