Carefully clean the fresh cactus paddles, using a sharp knife to remove spines and edges, then slice into 1/2-inch strips.
Bring a medium pot of salted water to a simmer (not a rolling boil). Add the sliced nopalitos and cook, stirring occasionally, for 10–12 minutes until the mucilage (gel) reduces and the strips turn olive green.
Drain the nopalitos through a colander, rinse thoroughly with cold running water, and pat dry with a clean towel to remove any excess slime.
Heat olive oil in a large skillet over medium heat. Add finely chopped onion and garlic, sautéing 2–3 minutes until fragrant.
Add the cooked nopalitos, diced tomatoes, sea salt, and black pepper to the skillet. Cook, stirring for 5–6 minutes, until tomatoes have softened and flavors blend. Turn off heat and toss in chopped cilantro before serving.
